Participant declaration

Every participant completes this declaration when they register for, or renew, their PVRSC insurance registration. It records that you understand the risks of playing sport, what the centre's insurance does and does not cover, and the rules you agree to play under. You complete it online on your Memberships page, or on paper at the front desk.

For a participant under 18, a parent or legal guardian completes it on their behalf.

Declaration

1. Warning and acknowledgement of risk

   I understand that playing organised sport — including basketball, netball and volleyball — involves inherent and obvious risks, and that I may suffer physical injury, loss or damage to property while participating in, training for, travelling to or from, or watching a PVRSC competition or program. I accept those risks.

2. Release and indemnity

   To the extent permitted by law, I release Penrith Valley Regional Sports Centre Limited, its directors, staff, volunteers, referees and officials from liability for any injury, loss or damage I suffer arising from my participation, and I will indemnify them against claims arising from my own conduct. This does not exclude any right I have under the Australian Consumer Law or any liability for the Centre's own negligence that cannot lawfully be excluded.

3. Medical treatment

   If I am injured or become unwell and cannot make the decision myself, I consent to PVRSC staff or officials arranging the medical treatment they reasonably consider necessary, including transport by ambulance. I agree to pay the costs of that treatment and transport. I have given the emergency contacts and medical information on my profile and will keep them up to date.

4. Insurance cover

   I understand that PVRSC holds limited group personal accident and public liability insurance for registered participants, and that my registration fee contributes to it. I understand the cover is limited: sports injury insurance in Australia can only reimburse certain non-Medicare expenses (for example ambulance, physiotherapy, dental or private hospital accommodation) and by law cannot cover any expense for which a Medicare benefit is payable, including the "Medicare gap". If I make a claim, I am responsible for any policy excess. I should consider my own circumstances and whether I need private health, income protection or other insurance. Details of the cover are available from the PVRSC office on request.

5. Fitness to participate

   I am medically and physically fit to take part in the activities I am registering for, and I will tell PVRSC immediately if my fitness or medical condition changes in a way that affects my ability to participate safely.

6. Rules and code of behaviour

   I will abide by the PVRSC competition rules, by-laws, code of behaviour and venue conditions of entry, including the Terms of Service at https://pvrsc.com.au/members/terms.php, and I accept that breaches may lead to suspension or exclusion without refund.

7. Privacy

   I understand that PVRSC collects and holds my personal information to run its competitions and programs, and may give relevant information to third parties such as its insurer, a governing body, emergency services or medical providers, as described in the Privacy Policy at https://pvrsc.com.au/members/privacy.php.

8. Photographs and images

   Photographs and video may be taken at PVRSC competitions and events. Whether my image (or my child's) may be used for promotion is a separate, optional consent on the portal profile, and I can change it at any time. Ticking this box does not grant that consent.

9. Fees and refunds

   I understand that registration and competition fees are refundable only as set out in the refunds section of the Terms of Service — including where I am injured and unable to play for an extended period — and that fees paid to a governing body on my behalf are not refundable by PVRSC.

10. Acknowledgement and agreement

    I have read and understood each point above and agree to them. If the participant is under 18, I confirm that I am their parent or legal guardian and I make this declaration on their behalf.
