A short note about your information

Penrith Valley Regional Sports Centre collects what you share here to keep an accurate incident record, learn from what happened, and meet our Work Health and Safety obligations.

What you tell us may be shared with on-duty staff, the GM, our maintenance team, and where required by law, with SafeWork NSW or our insurer. We keep incident records for at least seven years.

You can ask the General Manager to see or correct your information. Australian Privacy Principles apply.
About this report
Notifiable workplace incidents may need to be reported to SafeWork NSW within 24 hours. We may also share information with our insurer (Statewide Mutual) or icare workers compensation where a claim may follow. See our Privacy Policy.
